Online Wedding Registration Form

Below is the online wedding registration form, this is one of two forms Saint Andrew Catholic Church requires for performing the Sacrament of Marriage. The second form mentioned on our wedding page, is for the priest preparing you for wedding. Please have this priest fill out this permission form and return it to Saint Andrew Parish. You can download that form here. Please return the completed Permission form, along with your deposit. We will confirm the date and time in writing upon receipt of your documents. Additionally, you will need to request an original copy your baptismal certificates be sent directly to Saint Andrew.
